From the 91st day onward, CARPIMKO (the independent pension and welfare fund for nurses, physiotherapists, chiropodists/podiatrists, speech therapists, and orthoptists) pays an allowance through its disability and death benefits plan. The compensation is 1/730th of the average income subject to contributions over the last three calendar years (Article D. 622-7). It is due from the 4th day, for a maximum of 87 consecutive days (Articles D. 622-5 and D. 622-12), up to a limit of 360 allowances over three years. The applicable limit is €4,939.20. She compares the quotes based on what they cover: deductible, daily allowance, definition of disability, annuity, duration. She focuses first on the €1,814 per month that is missing from the 91st day, then on the €1,820 per month for total disability.

Result. Her 2026 CARPIMKO contributions, calculated on a base of €31,080 (€42,000 minus 26 %), amount to €7,294: €3,294 for the basic scheme, €2,704 for the supplementary scheme, €274 for ASV (Supplementary Old-Age Insurance), and €1,022 for disability and death benefits. We are using €34,706 as a reference taxable income (€42,000 minus these contributions; URSSAF contributions are not specified). In the 2026 tax brackets, this reference income falls within the 30 % bracket (from €29,579 to €84,577 per share), and the remaining amount after deduction is €29,767. A premium of €4,939.20 results in savings. €1,481.76 of tax (€3,516 before, €2,034 after): the actual cost is €3,457.44 per year, or €288.12 per month. This saving is precarious at this income level: it is worth 30% of the premium as long as taxable income remains above €34,518. If URSSAF contributions were to reduce it further, part of the deduction would be at a rate of 11%; in the extreme case where the entire premium were at 11%, the saving would fall to €543.31. The amount of compensation and annuity that the premium buys depends on the quote.

Hypotheses. The calculation assumes a constant benefit of €42,000, a single person with one tax unit, continuous leave without resumption of work or additional payments, and gross amounts: neither social security contributions nor income tax are deducted. The average annual income actually used by the CPAM (French Health Insurance Fund), calculated over the three calendar years preceding the leave, can be found in the personal online account of the Health Insurance system. Over 10 years from the start of the leave, with €20,251 the first year, €20,236 each of the following two years, then a total disability pension for 7 years, the lost income is €420,000, and the CPAM and CARPIMKO (French Pension Fund for Self-Employed Professionals) pay out €201,843: there is a shortfall. €218,157.

7. The group insurance contract (Madelin): what the deduction allows

Article 154 bis of the French General Tax Code (CGI) allows self-employed individuals subject to non-commercial profits (BNC), such as a self-employed speech therapist, to deduct contributions to an optional supplementary insurance contract from their profits. The Official Bulletin of Public Finances (BOFiP, BOI-BIC-CHG-40-50-40-20, No. 90) sets the limit: 3.75 % of taxable profit plus 7 % of the PASS, without exceeding 3 % of 8 PASS, with a floor of 7 % of the PASS. For 2026, with a PASS of €48,060, the floor is €3,364.20 and the maximum is €11,534.40. At €42,000, the limit of €4,939.20 lies between the two.

The covers to compare : daily allowance in case of stoppage, the deductible (aligning the deductible of the contract with the 90 days already covered by the CPAM avoids paying twice for the same period), the total and partial disability annuity, the capital in case of death.

The tax counterpart A premium deducted results in taxable benefits.
