Coverage is not transferred: it must be rebuilt.<\/p>\n<p>The French Overseas Social Security Fund (Caisse des Fran\u00e7ais de l&#039;\u00c9tranger) reimburses based on French rates. In a country where hospitalization costs ten times the social security rate, membership alone leaves the bulk of the cost to be borne by the individual.<\/p>\n<p>The three most costly breaks are not the price of the contribution: they are the medical questionnaire, the waiting periods \u2014 maternity leading the way \u2014 and the gap in coverage upon returning to France.<\/p>\n<\/div>\n<p class=\"wp-block-paragraph\">An executive who is moving to Dubai is preparing his holding company, his <a href=\"https:\/\/balmontconseil.com\/en\/expatries-mobilite-internationale\/structuration-internationale\/calcul-et-strategie-dexit-tax\/\">exit tax<\/a>, The sale of his primary residence and the taxation of his dividends. He devotes months to these subjects. Then, the day before leaving, he takes out his family health insurance online, in twenty minutes, using the first comparison website he finds.<\/p>\n<p class=\"wp-block-paragraph\">This is the most frequent reversal we observe, and the most costly. A tax arbitrage error can be corrected in the following fiscal year. An uncovered hospitalization cannot.<\/p>\n<h2 class=\"wp-block-heading\"><strong>What you lose by leaving, and what nobody tells you<\/strong><\/h2>\n<p class=\"wp-block-paragraph\">The rule is simple and brutal: <b>Except in the case of secondment, departure terminates affiliation to the French social security system.<\/b> The posted worker\u2014on a limited-term assignment governed by European regulations or bilateral social security agreements\u2014remains affiliated with France. The expatriate, on the other hand, leaves France. And the executive or freelancer who sets up their own business abroad doesn&#039;t even have to ask the question: they leave France.<\/p>\n<p class=\"wp-block-paragraph\">The second loss, the one discovered during the initial treatment: <b>French supplementary health insurance ceases to be useful.<\/b> Almost all contracts include a territorial clause that limits coverage to France, with at best an emergency extension for temporary stays. Moving house is not a temporary stay.<\/p>\n<p class=\"wp-block-paragraph\">Third loss, the most silent: <b>foresight.<\/b> Life and disability insurance policies taken out in France almost always contain the same territorial clause, sometimes with exclusions by country. A family might therefore leave believing that the death benefit of the head of the family will be paid out \u2013 when in fact it has remained at Roissy airport.<\/p>\n<h2 class=\"wp-block-heading\"><strong>The CFE is not a health insurance provider.<\/strong><\/h2>\n<p class=\"wp-block-paragraph\">This is the most widespread misconception. The Caisse des Fran\u00e7ais de l&#039;\u00c9tranger (French Nationals Abroad Fund) allows individuals to voluntarily maintain their membership, with a real benefit: it preserves their rights, it does not impose medical screening, and it facilitates reintegration upon their return.<\/p>\n<p class=\"wp-block-paragraph\">But <b>It reimburses based on French social security rates<\/b>. This mechanism is neutral in France, where the rates are set at these rates. It becomes negligible where the costs are completely different: the United States, Switzerland, Singapore, the United Arab Emirates, and Hong Kong. A hospital stay billed at $60,000, reimbursed based on French rates, leaves a significant out-of-pocket expense.<\/p>\n<p class=\"wp-block-paragraph\">Hence the two possible architectures, and a choice must be made with full knowledge of the facts.<\/p>\n<ul class=\"wp-block-list\">\n<li><strong>CFE + supplementary<\/strong> \u2014 The CFE (French Health Insurance Fund for French Nationals Abroad) reimburses at the French rate; supplemental insurance covers the remaining costs up to the actual amount. Rights are maintained, and there is no medical screening for the CFE portion. This applies to families with pre-existing medical conditions, long-term expatriations, planned returns to France, and countries with moderate costs.<\/li>\n<li><strong>First euro insurance<\/strong> \u2014 A single contract covers all costs from the first euro, without the intervention of a basic plan. Countries with very high costs, families without prior coverage, multi-country mobility, need for simple management.<\/li>\n<\/ul>\n<p class=\"wp-block-paragraph\">The choice is not based on the displayed price. It is based on three parameters that comparison sites do not display: the annual and lifetime ceilings, the contracted healthcare network in the country of residence, and the handling of medical history.<\/p>\n<h2 class=\"wp-block-heading\"><strong>The three most expensive breakups<\/strong><\/h2>\n<h3 class=\"wp-block-heading\"><strong>The medical questionnaire<\/strong><\/h3>\n<p class=\"wp-block-paragraph\">First-euro insurance is selective. A declared pre-existing condition can be permanently excluded from the contract, and an undeclared one voids coverage at the very moment it is needed. This point alone often determines the structure: a family with a known medical condition has no interest in leaving the basic CFE plan, regardless of the premium difference.<\/p>\n<h3 class=\"wp-block-heading\"><strong>Waiting periods, and maternity<\/strong><\/h3>\n<p class=\"wp-block-paragraph\">Maternity is a textbook example. International contracts commonly impose a waiting period of around one year before any coverage is provided. A family in their thirties who leave in January with plans to have a child within the year discovers in September that the delivery will be entirely at their own expense\u2014in a country where it can cost tens of thousands of local currency units.<\/p>\n<h3 class=\"wp-block-heading\"><strong>The return to France<\/strong><\/h3>\n<p class=\"wp-block-paragraph\">This is the least anticipated gap. The international contract ends upon return; re-affiliation with the French system is not instantaneous and requires proof of residence. In between, there is a period without effective coverage, which can range from weeks to months depending on the individual case.<\/p>\n<p class=\"wp-block-paragraph\">This period falls precisely at the time when the family changes countries, housing, schools, and often jobs. In other words, at the worst possible time.<\/p>\n<h2 class=\"wp-block-heading\"><strong>What we systematically check<\/strong><\/h2>\n<ul class=\"wp-block-list\">\n<li><strong>Departure status<\/strong> \u2014 Secondment or expatriation. This single point determines whether the French system continues to apply, and for how long.<\/li>\n<li><strong>Country of installation<\/strong> \u2014 Actual cost of care, existence of a mandatory local scheme, requirement of coverage for obtaining a visa \u2014 several countries make it a condition.<\/li>\n<li><strong>Medical history<\/strong> \u2014 Determine the architecture. This parameter takes precedence over price.<\/li>\n<li><strong>Ceilings<\/strong> \u2014 Annual and for life. A lifetime limit of a few hundred thousand euros is exhausted by a single serious illness.<\/li>\n<li><strong>Deficiencies<\/strong> \u2014 Maternity, dental, optical, chronic conditions. To be compared with the actual family project, not the theoretical one.<\/li>\n<li><strong>Repatriation<\/strong> \u2014 Often absent from entry-level contracts, or capped at a level unrelated to actual medical repatriation from Asia.<\/li>\n<li><strong>Insurance<\/strong> \u2014 Territoriality clause of French contracts, exclusions by country, maintenance of death benefit and disability pension.<\/li>\n<li><strong>Return scenario<\/strong> \u2014 Continuity of coverage during re-affiliation. To be contracted before departure, not upon return.<\/li>\n<\/ul>\n<h3 class=\"wp-block-heading\"><strong>Our position<\/strong><\/h3>\n<p class=\"wp-block-paragraph\">Health insurance for an expatriate family is not an insurance product: it is a component of the departure planning, just like tax residency or asset structuring. It is addressed <b>Before<\/b> the signing of the employment contract or the creation of the local structure, because it is at this point that everything is still negotiable \u2014 including with the employer, whose international group contract sometimes covers better, and sometimes much less, than it suggests.<\/p>\n<h2 class=\"wp-block-heading\"><strong>The calendar<\/strong><\/h2>\n<ul class=\"wp-block-list\">\n<li><strong>6 months before<\/strong> \u2014 Departure status, CFE arbitration or first euro, medical questionnaire submitted \u2014 it&#039;s long, and a refusal takes work.<\/li>\n<li><strong>3 months before<\/strong> \u2014 Effective subscription to clear any gaps before arrival. Verification of the employer&#039;s group contract. Resumption of supplementary insurance.<\/li>\n<li><strong>Initially<\/strong> \u2014 Termination or suspension of French contracts that have become obsolete \u2014 many families pay for two years for a mutual insurance company that no longer covers them.<\/li>\n<li><strong>Before returning<\/strong> \u2014 Continuity of coverage during re-affiliation.
